Payment in full for all office visits is expected on the day of your appointment unless you have applicable insurance that will be filed for your visit.
Co-pays, deductibles and co-insurance amounts will be collected before you are seen by the physician. Failure to pay your co-pay or co-insurance, or at least a partial amount, could possibly result in your appointment being rescheduled.
No-Show Appointment Policy
We recognize that everyone’s time is valuable, and appointment times for our patients are limited and important. Patients who fail to show for their appointments and who do not notify the practice in advance are costly, but more importantly, prevent another sick individual who needs to be seen from getting a preferred appointment time.
A patient who fails to arrive for a scheduled appointment without canceling the appointment prior to the schedule time is considered a “no-show”. Effective Feb 1, 2013, a $25 fee will be assessed for no show appointments. We ask that you provide 24-hour notice if you are unable to keep your appointment time.
We are providers of most insurance plans, including PPOs and some HMOs. We handle Worker's Compensation Personal Injuries. We are also participating provider for Medicare. Though we may not be listed in your insurance book, we may still be providers of your insurance plan. Feel free to call our Insurance Department at anytime, and a staff member will be glad to assist you. For more information, please call (601) 579-4440.
